“By the sacred anointing of the sick and the prayer of the priests the whole Church commends those who are ill to the suffering and glorified Lord, that he may raise them up and save them. and indeed she exhorts them to contribute to the good of the People of God by freely uniting themselves to the Passion and death of Christ.”

Canon law states that anyone who is in some danger of death from an illness or from old age can receive the Sacrament of the Anointing of the Sick. Likewise, should the person’s condition worsen, the sacrament may be repeated (cf. CIC, can. 1004). 
Moreover, If there is any doubt as to the seriousness of the illness or condition, the sacrament is to be received (cf. CIC, can. 1005). The Catechism of the Catholic Church also adds that it is also appropriate to receive the sacrament prior to surgery (cf. CCC, 1515). 

Homebound Ministry for the Sick: Only those who live within the parish boundaries will receive homebound ministry on a weekly basis. Due to Diocesan Policy, Eucharistic Ministers are prohibited from administering the sacrament of Communion to registered homebound parishioners outside of the boundaries of 22192. With that said, those who live outside of our parish boundaries may receive the sacrament of Communion from our priests on an infrequent basis (generally monthly and not on Sunday).
